Form Builder

Overview

At QNOPY, we know the importance of customizable forms. That’s why we spend so much time preparing forms for each client and their unique needs. This personalization is made possible by our Form Builder. The Form Builder allows QNOPY Admins, Client Admins, and Standard Users that have been granted access to build and customize forms for use in projects.

Accessing the form builder

Go to the Admin tab at the top of the portal, then click My Forms. From here you can see all existing forms. To create a new form, click Create Form.

AI Generating Forms

Forms can be generated automatically using AI. Upload a .jpg or .png file and QNOPY's AI form builder will build the form for you based on the image. To use this, go to the Form Library from the top menu and click 'Chat with Coco' in the top right corner. Form generation takes only minutes.

Editing a Form

To edit an existing form, go to My Forms, open the actions menu (hamburger icon) next to the form you want to edit, and select Edit Form.

You can only edit forms you have created. If you need to edit a form created by someone else, ask your company admin to make the changes or grant you the appropriate access.

Creating a New Form

1. Go to Admin → My Forms and click Create Form. 
2. Give your form a title — a form cannot be saved until it has a title and at least one question. 
3. Add tabs, questions, and elements as needed (see structure guide below). 
4. Click Save when done.

Form Builder Structure

When you open the form builder, the screen is divided into the following sections:

• Form header — Where you enter the form name. You can save or exit the form from here.
• Form tabs — Tabs appear directly below the form name. You can add as many tabs as needed, but each tab must contain at least one question.
• Tab settings — Settings for the active tab. This is where you can set a tab as a header or enable multiple sets for that tab.
• Form workspace — The main area below the tab settings where you add and arrange your questions.
• Elements menu — The right sidebar containing all available question types, split into commonly used and not-so-commonly used fields. Drag elements from this menu directly into the form workspace.

Adding Tabs

Click the + button to the right of your last tab to add a new tab, or duplicate an existing one. Each tab must have a unique name and contain at least one question.

Adding Questions

Drag your desired question type from the Elements menu into the form workspace. Then add your question text and any options or additional components required for that element type. Further customisation is available under Advanced Options for each question.

Advanced Options

All questions have the following advanced options available:

• Question header — Displays a header above the question in the form.
• Internal notes — Notes visible only in the form builder, not to users collecting data in the field.
• Guide information — Help text visible to anyone filling in the form.
• Required — Makes the question mandatory before the form can be submitted.

Visible Conditions

A visible condition makes a question appear only when a previous question's answer meets a specific condition. When adding a visible condition, select the question it depends on, the type of dependency (equal to, greater than, etc.), and the value that triggers it.

Math Calculations

Use a math calculation to display a value derived from a mathematical combination of other fields in the form. Select which fields to include, the mathematical operators to apply, and add or remove inputs using the + and − icons. Custom inputs can be added by selecting Custom from the dropdown.

Concatenation Calculations

Use a concatenation calculation to combine the values of multiple fields into a single displayed value. Select the fields to include and add or remove inputs as needed.

Conditional Calculations

Use a conditional calculation to display a value based on the answer to another question. Select the field the calculation should reference and define what values should display based on the answer to the field of interest.
